What is a Continuum of Care?
Mandated by the U.S. Department of Housing and Urban Development (HUD), a Continuum of Care (CoC) is a regional or local planning body responsible for coordinating housing and services for homeless individuals and families.
Responsibilities: CoCs track the local homeless population, coordinate community-wide efforts to end homelessness, and manage the distribution of federal funding to local shelters and rapid re-housing programs.
The Care Pathway: It ensures an individual's journey progresses logically from street outreach and emergency shelter to transitional housing and, eventually, permanent supportive housing.
